How fees work

What most people ask before booking a preparation call.

  • Insurance we work with

Fathom is out of network with insurance plans. You pay for each preparation call, block and follow-up yourself, and we do not bill your plan. We do not take Medicare or Medicaid. If you would rather see a clinician who bills insurance, we will gladly give you the names of colleagues who do.

    • Out of network with insurance plans

    • No Medicare or Medicaid

  • If you pay yourself

What you pay depends on what you book: a preparation call and a follow-up are 50 minutes, a block is three hours, and a package is blocks booked together. We give you your fee in writing before you start, so you hear it before you commit to anything. You pay by card, including HSA and FSA cards: at the end of a call or a follow-up, and at the end of each block. A package is paid when you book its first block. A small number of reduced-fee blocks are held back each year; ask how to request one.

  • Receipts for reimbursement

Each month we give you an itemized receipt (a superbill) with the dates and kinds of service, to send to your plan, which may reimburse part of out-of-network care. The receipt carries a diagnosis code your plan can see, and your plan decides whether it pays anything, so ask it before you start.

  • An estimate in advance

If you pay without insurance, you have the right to a written Good Faith Estimate of what your care will cost, before you start. We send you one before your preparation call and update it if your plan changes. If the final bill is $400 or more above the estimate, you can dispute it. The estimate is not a contract.

  • Checking your benefits

We do not check benefits, because we do not bill your plan. If you want to know whether yours reimburses, call it before you book and ask about out-of-network mental health benefits: how much, and whether a deductible comes first.

What a fee covers

What you pay for, in words

Your fee follows what you book, and you have it in writing before your preparation call.

  • A preparation call

Fifty minutes by phone or video, before any trauma work: your supports, what a block asks of you, the risks, and a decision together about whether a block fits.

  • A half-day block

Three hours with breaks. The fee covers grounding, rounds of EMDR, and a closing plan for the rest of the day. It follows a preparation call.

  • A package of blocks

Two or three blocks booked together, with the preparation call included, and a follow-up with three. It is paid when the first block is booked; its time limit and what happens if you change a block are in your estimate.

  • A follow-up session

A check-in after a block, in person or by video. Weekly or monthly sessions are billed the same way.
